			<content:encoded><![CDATA[<p>It used to be that bariatric surgery or stomach stapling was a rare and extreme response to dangerous cases of obesity.</p>
<p>But as more long-term data become available about the generally positive outcomes for bariatric surgeries, more consumers are opting for these procedures than for any other non-cosmetic elective surgery.</p>
<p>With most of us normally so reluctant to go under the knife, what&#8217;s driving the popularity of bariatric procedures? Quite simply, as an investment in health, they offer a very high return.</p>
<p>Follow-up research on patients of earlier bariatric surgeries is showing that the prospects for long-term weight loss are better than with any other mode of treatment for seriously overweight people, those classified as morbidly obese.</p>
<p>The data also show that post-operative health outcomes for patients of bariatric surgery are remarkably good, with major improvements in diabetes, cardio-vascular health, musculoskeletal conditions and sleep disorders.</p>
<p>But perhaps the most important outcome shown is that these health improvements are lasting, specifically because patients of bariatric surgery have excellent chances of maintaining a much lower, much healthier weight for years to come.</p>
<p>According to the Journal of the American Medical Association, bariatric surgery offers the best treatment to produce sustained weight loss in patients who are morbidly obese.</p>
<p>That assessment has been borne out by the statistics. The maximum weight loss is usually reached between 18 and 24 months after their surgery. The amount of weight lost tends to vary a little between the various procedures, but on an average, a loss of 60 percent of the excess weight was still maintained at five years after the surgery.</p>
<p>One study of over 600 patients reported that an average of 50 percent of the excess weight lost was still maintained at 14 years after the surgery. The numbers for weight loss maintenance in other treatment approaches come nowhere near these rates.</p>
<p>Roughly speaking, the morbidly obese are those defined as being 100 pounds overweight, or with a body mass index (BMI) of 39 or more. A score of 35 or more is used for those who already have associated health problems, such as diabetes or heart disease.</p>
<p>Also referred to as clinically severe obesity, it&#8217;s a condition that presently affects about six million Americans. And for these people, the risks associated with remaining overweight are simply much greater than the risks of going under the knife.</p>
<p>People at lower BMIs typically are not considered as candidates for bariatric surgery bariatric surgery without extenuating circumstances.</p>
<p>And sometimes people above a BMI of 40 are not considered because of other factors. Certain psychological conditions might preclude an individual&#8217;s candidacy, or an inability to commit to lifestyle changes necessary for weight maintenance.</p>
<p>For instance, people who consume lots of sweets and high-fat junk foods and who don&#8217;t really plan to give them up, won&#8217;t get the lasting improvements they want from the surgery.</p>
<p>And there are some patients who have delicate health complications such that they may have to lose weight in a supervised program before undergoing the surgery.</p>
<p>With rates for complications below 2 percent and mortality of less than one-half a percent, bariatrics surgeries have proven, safety-wise, to be comparable to other elective procedures, and far less risky than the heart bypass operations that are so common today.</p>
<p>That&#8217;s a remarkably low incidence of post-operative complications, particularly considering that the general health in this patient population is highly compromised by their excess weight to begin with.</p>
<p>Yet even as new techniques are developed that improve the safety and efficacy of bariatric procedures and minimize some of their unpleasant side effects experts worry that the public will get the idea that these are routine surgeries that offer a silver bullet solution to the problems of the gravely obese.</p>
<p>Surgery is a complex and demanding procedure for both the doctor and the patient, and like any surgery, it shouldn&#8217;t be taken lightly. Most bariatric surgery specialists will not agree to perform a bariatric procedure on a patient who hasn&#8217;t already demonstrated a concerted effort to get rid of some of their excess through more conventional weight-loss methods.</p>
<p>Surgical preparation usually includes weight loss and conditioning for the patient immediately before the operation, and then there is a lot of lifestyle and maintenance work afterwards.</p>
<p>That&#8217;s the investment. The return? Early death related to weight decreases markedly, and aside from the other specific medical improvements, studies also show that bariatric surgery patients get improved mobility and stamina, better mood and self-esteem, improvements in social interactivity and interpersonal effectiveness. And for overall quality of life, that means a real long-term gain.</p>

			<content:encoded><![CDATA[<p>I always feel that spring and summer are the most wonderful seasons of the year. I can do many things outside the house during those seasons.</p>
<p>Although I can also do my activities during autumn and winter, I find that spring and summer are more exciting, especially when it comes to burning off calories.</p>
<p>Here are some fun activities I do and the calories burned out of them. Maybe you&#8217;ll want to do the same. Check it out&#8230;.</p>
<p>1. Gardening &#8211; I clean my garden, water the flowers, plant seeds, trim shrubs, watch my blooming flowers, etc. Gardening during these seasons is fun and gives me lots of enjoyment. It is like a workout as I burn about 100-120 calories in 30 minutes gardening.</p>
<p>2. Bicycling -I love doing this activity. I can go anywhere I want as far as I can reach. This is like a low impact workout. But I don&#8217;t really go far, most of the time it is just in the neighborhood area. After riding about 30 minutes, I will have burned about 300 calories. This is a great way to stay in shape.</p>
<p>3. Trekking &#8211; I like to go trekking at a conservation park. It is fun and I enjoy the nice scenery; I also burn off some calories. For these activities I need more preparation because I have to wear a special outfit and need to do more warming up exercise to avoid cramps while climbing high area. For 30 minutes of trekking I burn about 350 calories.</p>
<p>4. Swimming &#8211; This is a fun way to stay cool during hot season. I go to the beach and indoor pools, depending on my schedule. However, I prefer swimming in the sea. It burns off my calories at about 300 in 30 minutes.</p>
<p>5. Water Skiing &#8211; This is a more rare activity I do. But, once in a while I go with my friends. It can help me build my upper and lower body muscles. From this activity, I&#8217;ll lose about 200 calories in 30 minutes.</p>
<p>If you have any other activities you like to do outdoor, just enjoy them.</p>

			<content:encoded><![CDATA[<p>Commonly called &#8220;glaucoma of the inner ear,&#8221; Ménière&#8217;s disease is related to the inner ear (labyrinth) and the balance system is contains.  The labyrinth contains three semicircular canals that together comprise a complex system of chambers and passageways in the temporal bone.  Signals sent from the semicircular canals travel along the nerve pathways to the brain.</p>
<p>Inside these canals is a membranous sac filled with a fluid called endolymph.  Surrounding the sac is another fluid, perilymph.  These two fluids bathe the vestibular and hearing organs, enabling balance and normal hearing.  When there is too much endolymph the condition is called Meniere’s disease.</p>
<p>This overabundance creates pressure, which builds until the sac bursts.  Then the endolymph and perilymph mingle, sending an unbalanced message down the vestibular nerve to the brain.  The resulting severe spinning, vertigo, nausea, hearing loss, tinnitus, and a feeling of fullness in the ear are the symptoms of Meniere’s disease.</p>
<p>As the disease progresses, hearing loss increases.  In 85% of cases one ear is affected; both ears are affected in just 15% of cases.  In half of all cases the attacks will subside after two years; in 70% of cases, they will disappear in eight years.  However, in all cases hearing loss may become progressively worse.</p>
<p>Medical treatments<br />
Although the exact cause of Meniere’s disease is unknown, we do know that there are various triggers, including stress, allergies, excessive salt intake, caffeine, migraine headaches, and barometric changes in pressure.  Common treatements of Meniere’s disease include prescribing a diuretic such as Dyazide, reducing dietary salt, and using of anti-vertigo drugs like Meclyzine (Antivert), Compazine, or Phenergan suppositories.  One of the best drugs for stopping attacks of vertigo is sublingual Ativan (Lorazepan) three times a day.  While these may help quell nausea and shorten the episodes, they cannot cure the disease.</p>
<p>Patients should also be skin tested for inhalant and food allergies, as 50% are found to have allergenic causes of Meniere’s that can be treated by injections.  If migraine headaches are found to be the cause, they are usually treated with medication.</p>
<p>Surgical treatments<br />
If medical treatment fails to stop vertigo, hearing loss, and ringing and pressure in the ear, surgery must be considered.  In the past the only treatment was total destruction of the ear, which cured the vertigo but resulted in total hearing loss.  In 1978 a different procedure was developed, one that cut the balance nerve to the brain yet preserved hearing and relieved vertigo in 95% of cases.  But because it involved operating near the brain, only the most serious cases were treated in this way.</p>
<p>Development of minimally invasive procedures<br />
With the advent of minimally invasive surgery for many surgical procedures, a search was made to find an effective office procedure to treat Meniere’s, one with minimal risk and discomfort.  One such solution is the MicroWick, a procedure that is not only minimally invasive but can be implanted in the office and used for patient self-treatment at home.  Patients place medication in their ear canal, and the MicroWick absorbs the medication and carries it to the inner ear.  With the MicroWick two eardrop formulations are used: antibiotic and steroid.</p>
<p>For patients that have persistent vertigo attacks, gentamicin (an ototoxic antibiotic) is used as an eardrop in dilute concentrations three times a day.  The patient is usually tested and seen once a week to titrate the amount.  The results of this treatment are good: while some patients need to have repeat treatments, 81% are free of vertigo for four years after treatment.  In some patients the inner ear is resistant to gentamicin and a vestibular neurectomy (cutting the balance nerve) or a labyrinthectomy is necessary.  Fortunately, this happens in only 6% of patients</p>
<p>For patients whose main complaints are hearing loss, pressure, and tinnitus, dexamethasone is used in drop form.  Dexamethasone is a powerful steroid that reduces inflammation, decreases allergic reaction, and may reduce fluid pressure in the ear.  With this treatment, hearing is monitored at two-week intervals during the course of the four-week treatment.  In many cases symptoms are relieved, and the risks and complications are minimal.  Rarely does hearing worsen with this treatment, but occasionally after the MicroWick is removed the ear drum fails to heal and requires patching in the office.</p>
<p>Vestibular neurectomy<br />
If minimally invasive procedures fail to alleviate vertigo symptoms and hearing is still present, a vestibular neurectomy may be necessary.</p>
<p>The original approach, the retrolabyrinine approach (RVN), was improved in 1985 to become the combined retrolabyrinthine/retrosigmoid vestibular neurectomy (RRVN).  RRVN offers immediate and permanent relief from the vertigo caused by Ménière&#8217;s disease in over 95% of patients, and preserves hearing in 99% of patients.  The procedure involves making a small opening just behind the mastoid and exposing a small portion of the mastoid bone.  This gives the surgeon quick access to the hearing and balance nerve, which he or she micro-surgically severs.</p>
<p>The surgery is delicate.  The hearing nerve and balance nerve are close to the facial nerve, which controls facial movements and expression.  To prevent facial nerve damage, a facial nerve monitor is often used.  This device warns the surgeon when he or she is close to the facial nerve.  In over 9,000 procedures utilizing this monitor, no facial weakness has occurred.</p>
<p>After the RRVN, a patient’s unaffected ear takes over the balance function, and vestibular and balance rehabilitation helps them recover quickly.  While vertigo is usually cured, hearing sometimes continues to deteriorate do to the progression of the disease.</p>

<p>Being intelligent matters. Despite the negative stigma given to intelligent people and so called nerds, society now understands that intelligence often counts more than muscle power, especially in this day and age.</p>
<p>Suddenly, being smart is cool. People want to develop brainpower these days. This means not just being smart in the sense of having a wealth of raw knowledge, but being smart where it counts, with practical application.</p>
<p>Here are five tips you can use to increase brainpower and boost your minds capacity to absorb and process information faster:</p>
<p>1) Choose supplements wisely<br />
Yes, there are supplements that actually work. These may include memory-enhancement pills or medications that stave off mental fatigue for adults; there are also IQ enhancement milk supplements for children. A good number of these supplementary products on the market can actually give children a mental edge and help improve learning. Both children and adults need to follow a good, balanced diet and avoid vices that literally destroy brain cells such as smoking, drinking alcohol, and using illegal drugs. This will help keep your mind sharp over time.</p>
<p>2) Try speed-reading<br />
This is a good way to bump up your learning curve. Speed-reading courses not only allow you to cover data faster, but they also train your mind to pick up visual details at a greater speed.</p>

<p>3) Test yourself often!<br />
Here is a good memory enhancement drill. A study conducted by several colleges separated students into two groups. One group was given 5 minutes to study a text, and was then immediately given three 5-minute tests. Another group was given four 5-minute periods to study the same text, but was not immediately given a test afterwards.</p>
<p>A week later, they were all given a test on the same subject. The group that studied only once but took three tests outperformed the group that had studied the text four times but had taken no tests. The results are clear: don&#8217;t try to learn by just reading. Apply the knowledge you acquire and test yourself repeatedly.</p>
<p>4) Do drills on some basic math drills<br />
Drilling basic math drills by using flashcards may seem like something that belongs in grade school, but even adults can benefit from it. Face it; most of us are spoiled by having calculators to do our addition and subtraction. At its core, math embodies the purest form of logic. Letting your basic math skills deteriorate is similar to letting your logical and deductive reasoning take a downslide.</p>
<p>5) Take Abstract Logic Tests<br />
There are test books available in bookstores that give extensive tests on abstract logic. These tests present one with sets of 3 pictures; the test taker has to extrapolate from the first three pictures what the fourth logical concluding picture will be.</p>
<p>Buy a few of these books and try them out! Abstract logic may not be a direct help in concrete learning, but it actually improves your deductive reasoning capacity.</p>
